The blame? Not playing intermediate jump overcalls here. I don't use them here, either. But, if there is a fault, it is that, IMO. The auction is easy after 1♦-3♣ intermediate. Advancer just bids 3NT.

As North I would consider no other action than 2♣.
As South though, 2NT would at least cross my mind. I don't think 2NT is terrible.

That's impossible. No one can give more than one hundred percent. By definition that is the most anyone can give.

Also no blame. I used to worry about finding a bid with hands like the south hand but that is just a loser long term in my experience. This happens sometimes.
