I win in dummy and play a heart to my King. If this wins I go for two diamond ruffs in dummy. If it loses I'll have to play for Qx and ruff only one diamond.

I would win in North and play a heart to the jack. If that wins, cash a top diamond and cross to a spade and play another diamond and if West has QJxx and East four hearts I am still home by ruffing a diamond in dummy. If the heart loses, I probably need more favourable red suit breaks.

Precarious contract.

I would try

T1:A, diamond discard
T2: 2. I expect East to have the A. If East plays low I play the K. This means I am down if East has AQxx and a singleton .

If K wins:

T3: A
T4: A
T5: to K unless East ruffs in
T6: ruff with 7

If diamonds are established from dummy. Otherwise club ruff, diamond ruff, K.

The line is similar if East wins at trick 2 with the A and forces us with a club

T3: ruff
T4: A
T5: A
T6: to K unless East ruffs in
T6: ruff with 7

If diamonds are established from dummy to K and diamonds. Otherwise club ruff, diamond ruff, K.

The early play suggest that West is 26 in the blacks and East 53. It is also highly probable that East has the A.

If East is 32 or 23 in the reds I think I am always safe, but if he is 41 my best shot is a heart to the jack. But lets check that still works with hearts 32 ...

Say I play a heart to the jack and it lost and West forces me with a club, I cash the ace, cross to a spade and lead a diamond up, If East follow I win and lead another diamond. East can overruff with the ten, but that does not slay me. If West blows on the third diamond, pitching a spade I ruff and play a heart - East wins his ace and West gets to ruff a spade, but that is still only 3 losers.

Obviously I know the hand, but a heart to the jack looks right. If that holds, cash a diamond, cross and play another diamond.

Why are we believing East's count signal in clubs? Declarer has shown at least eleven red cards
and dummy has three black winners to cover declarer's at most two black losers. Declarer will
obviously cash a spade if he has a losing club so it is pointless for east to always give count in clubs
unless he is trying to help declarer. I don't think we can assume east has three clubs.
