I don't understand a preemptive style in which you're opening crappy two counts with a 5 card suit
1. These hands are incredible rare. Why burden your system by increasing the range of your opening bids to account for this sort of crap.
2. What does 3H mean? Is south actually preempting with a defensively oriented 18 count? If so, I can only assume that this is some kind of tactical bid.
3. Once East has shown big hand, declaring 4♠ seems questionable at best. The 3♥ bid seems to have worked and I'd be more than happy to double 3N.

I think that I prefer 2NT to double, however, I still don't think I'm getting to 4♠.
Of the bids, I think that I dislike 3NT the most.
I think that this can really misfire badly.
West has a tough choice. I suspect that the hand is worth a lot more in Spades than in NT.
Even so, pulling 3N to 4♠ seems unilateral.

East, OTOH, is not as well-placed. His hand is much better than the hand on the other thread (balancing decision/3c), and it is hard to pass out 3H; but East still does not have a great potential source of tricks, and anything needed to be onside won't be. Plus, he might not get to dummy anyway!
Blame is clear. East did the deciding; and however reasonable, it was wrong this time and might be wrong over the long run.
If 3NT had worked out, do we have an "Assess the Credit" for an action a lot of us would not have taken?
