• Bids: _P = 10, _X = 8, 4♠ = 4, 4♦ = 3.
• Lead ♦K = 10, ♠A = 8, CQ = 4.

so, does the pass of 3♥ mean that partner has hearts? Or "not hearts"? or just "not 20-21 balanced"?

Not familiar enough with the Multi to know what to do here on any answer (besides "not hearts", of course), and when I have played multi, it's been the mini-multi, so pass will almost certainly disambiguate partner's hand with us. But I do think that's a very important question to have the answer to.

Certainly not doubling. They may be about to bid 5♣, which I would not double either.

Partner should pass 3♥ regardless of which suit he has, since telling the table is almost exclusively going to help the opponents who seem to have the waffles and the decisions to make.

♦K seems tempting at first sight, but declarer's failure to bid 3NT is surpricing. Probably he has nothing in spades and therefore likely ♦A, perhaps even with a side suit in diamonds.

Would opponents bid that way with 10 ♥s between them?
If not, partner has ♥. However, if 3♥ over 3♣ is 100% forcing (it should be), West might bid that way with very long ♥.
If you double and West runs to 4♥ and no DBL from partner, you could confidently bid 4♠ then. Nevertheless odds are heavily against that and partner most likely has ♥.
So the choice is between pass and DBL.
What is a mystery is why East bypassed 3NT with an unlimited partner. My guess is he is weak, distributional with a long broken ♣ suit, in which case 4♣ might not so easy to beat.
If partner has a singleton ♠ the ♠ ace lead may give you a lot of tricks, but what if declarer has something like 1=1=4=7. I will pass. It is anything but clear whether West will pass out 4♣.
Partner should still double a 4♥ bid by West even if you pass and he should not worry about a 5♣ contract. If he passes over 4♥ I will bid 4♠.
I would lead the ♠ ace against 4♣.
