The explanation of 2 is 5+, 4+, 12-15 total points.
Yes, it's best when he has 4, but isn't it common to hold your nose and make this bid with 3? If so, the explanation should say 3+.

The explanation of 4 is help suit slam try; 3+, 5+, 4+, 14-15 total points. Why 3+?

also it has 16 hcp. gib never opens 1nt with 5major, therefore it will have rebid problems, and lie no matter whatever it bids.

My partner and I also almost never open 1NT with a 5-card major; therefore, we adjust the explanations of NT rebids so-as to not lie and mislead both partner and opponents. If the explanations of 1-1-1NT and 1-1-2NT leave no gap, there is no lie. This simply means that those two ranges are bigger than the ranges for 1m-1-1NT and 1m-1-2NT, because the former includes 15-17 and the latter doesn't.

Raising with 3 is considered a "least lie", but it's still a lie because the bid ostensibly shows 4-card support.

The same with the slam try. It's usually a 3+ suit, but sometimes there's no such suit that needs help.
