Bidding was:

P 1 2 5
5 6 AP

Apportion the blame between North and South. The 1 opening was standard, i.e. either 4+ cards or 4432. If 5 was wrong, what should a 5 bid look like?

I think the 5 bid is ok. If anything, the hand is a little too strong.

So 100% blame to South.

What was south thinking, 2-2 in the majors with the king of spades over a passed hand bidding 5? Just pass and let north crack it.

Btw south should also open 1NT and I don't think it's that close.

South made a mistake of bidding 6D. I don't know if this auction creates FP for NS, (it shouln't because responder was a passed hand) but *if* it does, then Dbl is automatic, warning partner not to bid more. If it doesn't, I'm torn between Pass and Dbl but I would never do the solo 6D with this type of hand = bad distribution, bad majors, badly placed SK.

3 bad bids

1 opener (how the hell is this not a 1 NT opener?!?)
5 bid by north (why aren't you asking for a spade stopper again?)
6 by south

I'd say 95% south 5% north. Both of south's actions were clearly wrong while 5 by north is a meh bid but isn't clearly the wrong bid (imo).

5 is not a strong bid, it clearly wouldn't set up an FP even by a passed hand.
