I'd consider a heart then lead a club...Q9xx is a much worse lead than QTxx so I agree with that being "fourth best".
Would anyone change their answer if the heart ten was, say, the seven?

I agree with those who hate a heart lead (whether Tx or 7x) against 3NT.
Different story against 1NT, I would probably lead the doubleton heart if the auction went 1NT all pass.

That's impossible. No one can give more than one hundred percent. By definition that is the most anyone can give.
