We open 2N with AKT8x K9 AKx QJx.

2N (P) 3D (P) : 3H (P) 4C (P) ?

3D = transfer

Does 4D deny 3-card heart support?

4C showed 5+ hearts and 4+ clubs
Your options over 4C are theoretically:

4D - either a very good hand for hearts, or a cue bid for clubs
4H - preference for hearts
4S - clearly agrees clubs (I think)
4NT - no real interest in either hearts or clubs

This particular hand is a 'slow 4NT' call: you have a very pretty hand, but you don't have a fit for either of partner's suits. If you bid 4D and then 4NT (natural) over 4H, partner will think you have a club fit.

I would actually interpret the options backwards from FrencesHidden.

I would take 4 as club agreement and asking, 4 as heart preference, 4 as heart agreement and asking, 4NT as to play.

IMO, 4 as "either-or" is too hard to work out. As a heart "Last Train" type of call, it is slicing the pie too thin. As a club cue, it is describing a leaf on a tree when a forest is unknown and unknowable. How to unwind this next is a mystery, as Responder does not know the focus suit.

We superaccept with a 3c. This has the disadvantage that you can't stop under game (or only wit a mis-fit) after a transfer.
But it has the adavntage that any bid now , except 4NT, shows a -fit.
