- I have nothing in hearts and don't want to bid 3NT
- I have a sexy holding in spades, but not a good enough hand to bid 3S, which shows 1 of the top 3 spade honours plus 1.5 cover cards elsewhere

The reason I posted this is because a few years ago I swear a lot of you suggested making a 4♠ call with a hand like this?!
3♠ was reserved for limit raises, which seemed amazing to me.

No. Some people advocate bidding 4S with the crappy 3 card raise hand while bidding 3S with the 3 card LR hand, but that doesn't mean 3S specifically shows a 3 card limit raise as it is a doubleton spade like 90 % of the time.

Actually, though, it depends on what everything means.
1. Is 1NT forcing (or semi-forcing), or is it standard?
2. Is 3♦ GF, or a 5-5 intermediate?
From the answers so far, I'm assuming that 1NT is forcing/semi-forcing and that 3♦ is a simple GF JS.
If so, I agree that 4♠ should be the P.O.S. 3-card raise.
I think 3♠ should be, as Justin indicated, 2-3 card support with relatively unlimited values, except probably not a stiff heart, although I'm not sure if 4♥ agrees spades or diamonds. I'm also not sure what 4♣ should be, but I suppose it is a power diamond raise. That's how I would take it.
3♥, then, seems to be (yuck!) natural.

3♥ shows:
[A] a 3NT bid with either no unstopped suit, or hearts unstopped;
[B] a good hand for either spades or diamonds or both.
3NT shows a 3NT bid with clubs unstopped.
Over 3♥, 3♠ denies a heart stop.
This kind of thing has wider applications. For example, over 1♦-3♦, 3♥ shows:
[A] a 3NT bid with either everything stopped or hearts unstopped;
[B] a slam try in diamonds.
3♠ is a 3NT bid with spades unstopped, 3NT is a 3NT bid with clubs unstopped. Over 3♥, 3♠ denies a heart stop.
