It works a lot more than it should however. I'd much rather have the hearts on my left by the way.

At these colors, LHO never has AKJxxxx and, even if he does, he may not lead them.
Of course, you could still get a heart lead when the top heart honors are split, but there is still a chance that the hearts are blocked or that partner has a stop (Txx may do).

I suspect that 3NT is the right contract, but not from my side of the table. So 3NT is out.
YUCK!
Double is out, not so much because I have only 3 spades, but because I have only 2 diamonds.
I bid 4C and hope that we can land on our feet.

If preemptor has AKJxxxx, there will be no outside entry. So, there will still be a high chance of scoring your Q♥ as there may be a duck to preserve communication with partner.
Double doesn't work very well if partner is 3-3-4-3.

I hate the do-nothing bid of 4♣. Where are we going, opposite a passed hand? There is virtually no way to reach a making game after this... maybe with KQxxx xx Axx xxx he can bid 4♠ along the way, but that is a tiny target. Our hand is so soft that it is very, very tough to picture a hand opposite which we make 5♣, unless he has a stiff ♥ and a maximum pass.
I hate pass: there is no realistic hope of partner balancing, especially when he has the cards to make 3N a legitimate contract.
I hate double: I can stomach the short ♠s, but not both the short ♠s and the very short ♦s... and my ♥ holding reduces the chances of his bidding 3N over my double.
I hate 3N. Not only do I need to score a ♥ trick (which I may do even when we have no stopper) but I need the ♣A just to get to 8 tricks!
So I need not only help in ♥s (from partner or the opps) and the ♣A but also the ♦A or the ♠K... or a blockage of some nature in the ♥ suit.
And to make things worse, even if LHO refuses to lead a ♥, because he's seen his partner's 3rd seat favourable preempts before...they may beat me by leading diamonds!
Finally, on most hands where we don't catch the ♣A in dummy, we're going down a LOT... we could easily be down 4 or 5, altho we ain't going to be playing 3N doubled
Okay, I'm a lot braver on a forum than I am in real life.. I bid 3N, with supreme confidence.

West: ♠ 75, ♥ AJ8532, ♦ QT2, ♣ 32
North: ♠ AJT, ♥ Q4, ♦ K3, ♣ KQJ987
As you can see, 3NT would not have been a big success. What does one hand prove? NOTHING But I thought that this hand was very interesting.
