When the current host leaves a table what determines who the system makes host next?

The player that joined the table earliest becomes host.

All too often, NO ONE becomes host for several hands.

So persistently slow players can't be ejected.

There is always a host. Not necessarily a player.

The behaviour you are observing is consistent with the host standing, but not leaving the table.

When a host leaves, the new host is assigned immediately.
