Missed Opportunity

I swear that it was my plan to end play RHO with D and grab 2 more H. But missed to count that 1 H I need to duck further in end game.

No need for fancy squeezes or endplays. When you played the second round of spades, you could have just played the ace and another to force out the king. Now you have 9 tricks (2, 2, 2 and 3).

I should have bid stronger on board 1 -- with Kxx in partner's suit, my hand is too good for just the 1 rebid.

3H is still possible on round 3 to show a supermax or that you forgot to jump shift.
... and I can prove it with my usual, flawless logic.
