This is a disadvantage to playing 1=4+ instead of 1=5+; I think if 1 showed five it would be a fairly clear-cut (although not without risk) bid of 4. Hopefully your treatment of 1=4+ will show compensating gains on other deals, although given that you don't play a strong club system you might want to reconsider the method (there are a lot more "problem hands" opposite a potentially short diamond).

I agree, our 1S showing 4+ was to blame here and your comments have given me more to think about. This is the first hand I have seen this problem and until now have thought the advantage of 1S=4 and the information that double denying 4 spades carries has been very beneficial/

I think South should double 4 for take-out. Pass is not terrible though, but this is the kind of auction where stretching a bit can work in more than one way, and is rarely a disaster.
