I don't think partner should ever be 1345. If his clubs weren't good enough to open 1♣ he should bid 2NT, not 3♣.

WTP pass, I cannot see how 5D can be better than 3Nt more than 50% of the times and i dont like my hand for slam I dont think its close.

Doesn't D:AKxxx + C:KQxxx put us on a SQ finesse for 6D?
Would partner get slam-uppity with HK added?
Now it's on only losing CA.

If partner has x Kx AKxxx KQxxx he would not bid 3NT
If partner has x xx AKxxx KQxxx slam needs the spade finesse plus clubs 3-2 (minus singleton ace of clubs)
I don't see any 55% slam opposite a 3NT bid.
