is standard here some kind of picture bid with primary or secondary support for M, or is this a slammish single-suiter? If the latter, is it demanding cuebidding or suggesting? And is 4N to play?

To discuss this, please specifiy, if 3D instead of 4D could be passed.

I am not saying it is standard, but in some 2/1 systems the 2/1 response
is GF, unless responder rebids his suit on the 3 level.

If you have clarified this, you know to show a stong single suited hand as
responder, from this followes than what kind of hand 4D cannot be.
If 3D showes a single suited GF hand, you dont need 4D for this anymore, hence
4D could show diamonds and hearts.

Next to clarify would be the meaning of 2NT, it makes perfect sense to play
2NT as split range, either as 12-14 or as 18-19.
If one does, responder would be better served, if he responded with 3C - kind
NMF style, to find out, what opener has.

The advantage of a 3C NMF style bid is, that opener would also be able to show
3 card diamond support.
Something responder may be interested to hear, if he is looking for a slam.

Come to think about it, the only hand responder will be hard pressed to show,
is a single suited hand with diamonds and heart shortage, i.e. the 4D bid would
ask opener how big the wastage in hearts is?

Since 2 is game force, I would think that a slammish one suiter would rebid 3, not 4. Also 3NT is bypassed, so perhaps 4 could be used as a fit-showing control bid. But in truth I don't know if there is a "standard" meaning for this.

2N is split, 2D is 100%GF

I would take it as slamish one suiter, asking for controllbids.
3 would be diamonds, but still with doubt about the strain, 4 sets trump.
3 M would be slammish with partners major.

But as usual I have no idea what is standard.
