unable to replace ...red player replacement refused

when running a tournament...what cause one not to be able to replace a timed out player(red)?
usually the error is unable to replace, ....refused.
people at the table want to play and sometimes it takes several tries to get a sub to replace the player

TD has the option to do this or even BOOT players

Maybe the invited sub refused? Also, "unable to replace" may occur when you invite a player who already plays somewhere in another tourney. Even if he accepts, substitution will not succeed.
