I think N could safely double 3nt again. If partner takes it as a stronger hand and take out, that is fine. If partner takes it as penalty suggesting a spade lead, that is also likely fine.
You probably don't find the game but at least you should get +100 instead of +50.

They will play 5D anyway and its going to be tough to bid 5H without taking a big gamble.

North could double 3NT, but where should trick 5 come from?

North has a clear double over 3NT IMO, especially when red vs white.

East made an interesting bid. I don't blame Nth for not doubling at all, Swop the 2 and the T of S and I would double. As it stands, Nth might well be conceding a doubled game.
