How E can stop below game after hearing 2 is beyond me so it's either 100% his fault or there was misunderstanding as to 4 call.

East should not try for slam opposite a passed hand, so he can just jump to 5 over 3 and not risk a misunderstanding. Even if West thinks 4 is non-forcing, he has a supermax opposite a partner who showed extras twice.

Yeh, I would have thought so, too. Opener showed clubs, spades, and maybe then was trying a probe for 3NT with 3D. How else could he then retreat to what they can make? Pass 3H?

At some point (like after 3H) East has to make sure game is bid by bidding game.
