I blame North for not showing his second suit and for getting partner to a 5 lvl game with a void (though I know it can be right at times)
Edit: I didn't quite realize North can't bid his second suit naturally, but I still blame him the most.

Anyway, the worst bid was West's failure to double 5♣, the second-worst the raise to 5♣.
Btw, I disagree with Josh, West seems to have two double cards
