Running from 3NT

3NT shows 2-5C; 2-5D; 2-3H; 2-3S; 13-15HCP; partial stoppers in C/H/S.

What is supposed to happen after North runs from 3NT? Is South supposed to think his partner is trying to find a better game, or to investigate slam? If he's just trying to improve the contract, should 4NT be Blackwood?

4 is explained as 3+D; 4+H.
6 is explained as 5+D; 5+H; 3-card D.
In addition to the D inconsistency within the 6 explanation, how did the distribution change, when all North did was answer keycard questions?
