I would expect it to be asking for a club lead if we didn't have any prior agreements.

I would take it as command to lead a ♣ instead of trying to footsie around by leading some other suit and hoping to get pard in for a lead through declarer's presumed stopper...

I learned this to be a club request many years ago, and I have never heard anyone argue otherwise, so I'd expect it to mean this if perpetrated by my partner.
I can see an argument that it means guess a major, since he can't really make a bid over my preempt with AKQJx or the equivalent, but I don't think it is normal. I'd guess partner has unexpected club stuff (Qxx?) and a diamond stopper, with some major smatterings.

I believe the standard here against NT after a lead directing double is to lead in the following order
1. lead my suit (when we both have bid)
2. lead your suit
3. lead dummys first bid suit (even when artificial, when we havent bid)
4. lead diamonds (against 1NT-3NT)
