Fairly sure I would duck the lead, ruff the continuation and play ♣K pitching a diamond. Ruff the presumed diamond continuation and cash a top trump. Given the info in the OP i'd probably try heart hook, cash the ace and play trump to hand to cash clubs. I'll make when LHO is Qxxx Kxx in the majors.

I've gone off before though.
