I'm not going to play small at trick 1, that's for sure. When they lead a club in this sequence, I don't expect them to be divided peacefully Hxxx-Hxx. It will often be a singleton.

I think I would play a spade from dummy at trick 2. The communications are bad. The worst thing that might happen is that east rises with the ace and gives partner a ruff, but if west has xxx or Jxxx in trumps that's not a problem.
Alternatively I could draw trumps and take stock, hoping to escape with one diamond loser.
