It didn't happen to me, but I saw it in the recaps when reviewing my hands in a Robot IMP Duplicate.

Thr human opened 1NT, West bid 2♦ showing majors, and North bid 2NT Lebensohl with 3=5=2=3 and not much strength. The human bid 3♣ as commanded, and they played in their 3-3 fit, doubled, going down 3.
