If LHO has 4♦ he is either 3♠3♥4♦3♣ or 4♠2♥4♦3♣ 12-14could, that leaves RHO with no more that 2 points.
Finesse ♦J; Then cash your high ♦ and dicard ♣ (unless RHO doesnt discard 2 ♣)
If RHO discards at least one spade, ♠Qx isnt dangerous anymore, ♠A, ♠...
If he discards 2 ♥, play ♥A, ♥ to ♥T establishing ♥. Actually you will win even if LHO has 4♥. Unless ♠Kx, ♥Qxx, ♣KQx, ♦Jxxx that most probably isn't an opening or ♠Kx, ♥QJxx, ♣KQx, ♦ Jxxx with brilliant lead.
If LHO discards ♣&♥ play ♥A, ♥ to ♥T. Either RHO had 4♥ and you establish ♥ or he had 5♥ and you will get spade.
If LHO has 5♦, use same idea, ♦T, ♦A and look onto discards.

If RHO discards one ♠ and 2 ♥, most probably he has sth like ♠QT9 ♥Qxx left. Endplay him: ♠A, ♥A, ♥K, ♥.
