Pass - Pass - Pass - 1
2 - Pass - 2 - ???

The 1 opening is regular 2/1. The 2 bid shows 5-5 (or more) in the majors. What do opener's double and 2NT calls mean here? What is the right call with 18/19 balanced? How about with 4-5 or 4-6 in the minors?

I would play 2N as natural and X as takeout. Possibly not best, but pretty natural at least.

Indeed. I have specific agreements here, but opposite a random pard, would take 2NT as nat.
