I would say that the second hand is significantly more likely, for 2 reasons.
1) West has come in with 2H in an auction that smells of a misfit with partner passing, both opponents bidding, and North being reasonably likely to have 4 Hearts stacked over you for his earlier double.
2) If West held an entry outside Spades he would be more likely to lead Spades from the top down rather than underleading AKQ
