I would try something like a dummy reversal, so I ruff a diamond and play the queen of club, a club to the king and they...?!
Kind Regards

Not sure at wich point you play this Roland, but ♣Q is ducked and LHO plays ♣10 on second round, East takes the club ace and returns a club.

I tried so far:
Ace of D
D ruff
Queen of clubs
club to the king losing to the ace.
now I discard a heart on the club return.
This leaves
♠ AJx
♥ Q
♦xx
♣ J9 (x)
♠ KT8x
♥AJ9x
♦-
♣-
lefty has not yet played to the 5. trick, he may or may not ruff....
1. Lets say, he follows suit or discards. I ruff a diamond with the eight and he...?
1.1. When he does not overruff the diamond I have won 5 tricks from 6.
Now I will try the ace of heart and ruff a heart small, then I ruff the diamond with the king and a heart with the ace. Now I have nine tricks and ruff a club with the ten so that the jack of spades will be my tenth trick.
1.2.1 When he overruffs the diamond and returns a heart, I have a free finesse. I win the queen and ruff a diamond with the ten.
What shall he do?
1.2.2 When instead of a heart, he returns a trump, I have this situation:
♠ AJx
♥ Q
♦x
♣ J9
♠ KTx
♥AJ9x
♦-
♣-
When trumps are 3-2 or 4-1, I can draw all, ending in dummy, claiming ten tricks.
2. When he ruffs the club and return a trump, I let it run to my hand and draw trumps, ending in dummy.
When he ruffs and return a diamond, I will ruff and play King and ace of spade.
Then I can play high clubs till they ruff with the last trump and win the rest.
What to do with trumps 5-0, I do not know.... yet.

Sorry Roland, I wrote the outcome, but somehow I forgot to send it an loed the data.
your line works wonders:
This was from the match against China National Women's team (olympic champions, and now also the NEC cup champions), at our table north opened 1NT and played 4♠. After ♦Q lead, she run ♥Q, ruffed the ♦ continuation, played ♠A+♠K to test trumps, and when ♠Q didn't appear, she ruffed a heart in hand and tested if hearts were good.
Hearts weren't good, but even when north tried the duck the club, he had no diamond for escape, so sooned or later he would have to give a second club trick to declarer or a marked finese against his ♥10
At the other table, ♥Q was run, and ♦ ruffed, now came ♣Q wich was ducked around.
declarer coul still succeed had he continued clubs, but instead a heart was ruffed and declarer tried ♠A + run ♠J, defence took ♠Q, crossed with ♣A and a heart came back ruffed with last trump, ♥9 was still to lose, so down 2.
