say you play 2M as promising that suit and another.

what should 2M-(3x)-X mean? p/c or just plain penalty?

In the country of the Muiderberg convention, it is standard to play it as penalty.

I used to play it as p/c over 3♣ but penalty over 3♦. However with a (51)43 opener must pass 3♣X.
