Too good to pass, and 4NT won't help much: if partner tells us 1 keycard is missing we have to just bid 6 which may have no play. After 4♠ sometimes partner can keycard, or else he can make a useful cuebid. We can bid 5♦ over 5♣, or 6♥ over 5♦.

Partners 4H may be made with lots
of hands, and it is not even be certain
that 4H makes, even with the hand
I hold, although it would be unlucky.

I am not happy with 4♠ since the folow-up auction is going to be nebulous at best.
My instincts tell me to bid 4NT. If we have all of the key cards, I will try for a grand. If we are off one, I will bid the small slam which may not be cold, but should have play. If we are off two key cards, I will bow to those who passed 4♥.
A grand will make opposite AKJTxx of hearts, ♠Ax or Axx and the ♦K. Certainly well within the normal expectation for a 4♥ bid. A small will make on most 4♥ overcalls. I am sure that one can give partner a reasonable 4♥ call that is not safe at the 5 level, but one would have to work at it.

I am only invitational to slam so partner is going to have to make this decision.
Although i can imagine it being hard for her as ace, ace, trump queen, spade shortage are a lot of key features.

If partner has something similar to that - say xx A10xxx K KQxxx - we might find that 5H is too high.
