I blame the system most of all, if 3D is not game force (no specific agreement tells me not enough). But if 3D is game force (and north's bidding suggest it was), then given the options above to assess the blame, I blame responder the most, simply because he took control when he didn't know where he was going, and he used up a lot of space with his bid of 4D. Over 5♠ he worried (I guess) about being off two top clubs to explain his pass.
If 3D is game force, responder has a variety of bidding options. I would rebid 3S to see what opener does, or perhaps bid "serious 3NT" if playing that. Let's look at two simple auction, on offering a choice of strains (the second one) and the first making a waiting bid of 3S (also denying heart control).
1♣ - 1♠
3♦ - 3♠
4♣ - 4♦ (dbl)
Pss - Rdbl
4♥ - ?
Now respnder can bid blackwood (thanks to heart and club cue-bid) and not worry about being off two top clubs. Or, a similar auction.
1♣ - 1♠
3♦ - 4♣ (club support, some slam interest)
4NT - 5♥
6♠or
6♣
Obviously, there are many ways to get to slam not using 3D as a splinter here as well.

One player used RKCB and couldn't diagnose whether or not there was a slam after the response.
Hence RKCB wasn't the correct bid.
Hence it was all his fault.

Responder is 100% responsible because RKCB should NOT be used if the answer you get won't tell you what you NEED to know to make a good decision on going to slam or not

50-50: opener shouldn't pass on 4♦-(Dbl)-?, but ask for keycards right away himself; and responder knows his partner has a ♥ control, so he should bid 6♠ with his doubleton ♥ and all other suits covered...

I agree and also put some blame on system. That's why I play 3
as a 14-15 HCP (roughly)hand with good 
's and singleton .That leaves 4
for a 18-19 HCP(roughly) hand again with good support. Think that makes life a little easier.Mike
