Prefilled word/excel standard conv card

Where can I find it?
From www.ecatsbridge.com, I could only find World standard card (with notes only, but not the CC), or WBF Generali Card and Notes (with card and notes but PDF).

I would like to get a prefilled word/excel (word preferrable) standard card to start with to make one.
