By agreement 0445 are opened 1♣, so they are a bit out of the picture.

To me 3♠ is choice of games, in the context of knowing partner will have only a singleton or a void of support. 3N says opener prefers 3N, nothing else. Of course this has to do with the number of spades, but also with the type of honors held, etc.

0 would be typical, though there are definitely hands where 1 is possible.

When I found this problem I bid 3♠ with a very strong 5233 in order to find if I should play 6♦ or 6NT, I missunderstood my partner and finished on a stupid 4-3 fit where 6NT was laydown.

Gonzo: I would think 4N quant and 5N pickaslam would be options with your hand.
In answer to your question, 0 or 1 spades are possible I think. 3H may just be a hand that wants you to play NT.

0-4-5-4 is most likely in my style, but remember that I frequently rebid 1NT with 1-4-4-4's. I know that you (=Fluffy) dislike this so then 1-4-4-4 is definitely an option. As Arend notes, opener is not forced to bid 4S with that shape if his honors suggest notrump.
Please note: I am interested in boring, bog standard, 2/1.
