I think that the problem is missing one pass.

Isn't partner the 3H opener?

In that case I would bid 5D.
Please note: I am interested in boring, bog standard, 2/1.

I interpreted it as RHO opening 3H, LHO bidding 4H, partner bidding 4S, passed back to us. Oh you're right since "I would not have bid 4H" is an option. If there is a pass missing, I would bid 5D now too. Risks pushing them into slam, but will probably get us our best lead and maybe they'll sell out.

I interpreted it the same (opps bidding and raising hearts), and passed.

Writing auctions in several lines of exactly 4 bids would help A LOT! I suppose there's one 'pass' short, but I already voted as if I was the partner of the 4 bidder...

About the correct bidding: I'll bid 5.

I am bidding 5, but I disagree with my 4 bid.
maybe in a pick-up game, but with regular pd's our first and second seat preempts tend to be decent.
Give pd something like AKxxxxx and stiff and we have nice slam, but maybe I am just dreaming.

Partner did not preemp, but the void
in his suit is a downgrading factor, i.e.
pass.

Hmm, I see that not so many people pay attention to the first question asked. How to respond to pd's preempt.
He opens 3 in first seat, followed by pass of RHO and you bid 4, LHO bids 4 and pd passes and so does your RHO.
Now it is your turn to bid, or pass or do whatever you like.
And there are no passes missing.
