Incidentally, I'd take 4NT here as showing great heart support, with Qx of spades.

Is 2NT just negative or second negative? What is the strongest hand I could have to make the 2NT bid?

I would bid 3 at my second turn. Reasonable suit, if we belong in NT, I am telling where my strength is and better played on his side. Assuming I bid 2NT for some reason, now I might bid 4 only, thinking of 5H though.

4 could be a choice, too. With that garbage hand, I wouldn't bother to introduce my suit, so bid 3 instead.
