From GIB's notes here it says "When leading from 3 small, it leads top of nothing against NT, low against a suit contract."

Yep, I discovered that a while back as well, plus a significant number of other exceptions. (Never followed that up with a full no trump listing, since I couldn't really come up with any rules regarding leads from multiple honors)
