A difficult declarer problem

I would play West for AQJx or AQxx or AJxx A and QJx(x)

If spade Q denies K, I can not afford to duck, E will cover and play for down 1.
So spade A, Dia AQ discarding a spade, and spade ruff to hand.
Play towards dummy. LHO will win this probably. exit diamond, I will ruff in hand.
Play 2nd to dummy, and play 3rd and ruff
Go to dummy with 3rd and play 4th .

a-East has the 4th spade, then I ruff and play T, hoping West originally had AQJxx , endplaying him for 10th trick
b-East shows up on 4th spade, then I simply discard a club and endplay West again.

If it turns out that East has 2 hearts and 4 spades, then I will have to play W for holding originally Jxxx

I think my suggested line above fails if West exits with 2nd (if he has one) instead of or

to dummy. Say K wins. Ruff 3rd .
to dummy. Say West wins and

1) Returns S. Ruff. Return to dummy with trump and ruff , exiting with 10.
2) Returns D. Ruff. Back to dummy and ruff , exiting with 10.

Not making if A is with East.

They will not duck first unless you bribed them.

Let's hope West holds the stiff A!
More realistically, West's 4=2=2=5 or 4=1=2=6 should get us home.

So I tried a legitimate line of endplayin west with 4th spade, sadly he was 4234 and rightfuly escaped with second heart after winning A. This meant int he end position he had a third diamond I couldn't ruff out to escape endplay.

At the other table spade lead was ducked, and East, who doesn't have K10 decided to duck in fear of setting up a trick in spades for declarer.

That's what happened at our table. South opened a multi and declared 4H. I led the SQ, and my partner with Kxx of spades was worried about overtaking and setting up an extra spade trick if declarer started with 10x; he couldn't see there was any need to overtake and play a club - the contract was either cold or one off, unless South had 7 hearts for his multi.

This hand is a good reflection of how it may look obvious to you how the defence will go (if you duck RHO will overtake and play a club), but it's not at all obvious to the defence.

Teammates stayed out of game, so that was a painful swing out.

In this case I now have the feeling I played badly, but the problem with legitimate vs ilegitimate line problems remains the same, you can't analyce afterwards which one is better.
