Close enough that I ask for the form of scoring, though I'm not yet sure that influences my answer.

EDIT:
Now that I've thought about it,

I lead 5 at IMPs
I lead Q at MPs at a tournament
I lead 5 at MPs at the club, because it's more likely contracts will vary, and also because if contracts are the same, I have good chances of gaining later in the play and so should just make the field lead to reduce variance.

Partner couldn't chirp 1, the 1nt bidder doesn't usually have a 4-card major (unless it's me with a 4 triple) makes the Q obvious imo.

This isn't at all clear to me. Any suit (including a D) could work out best. If my hand was weaker (with the CJ instead of the K for example), I would lead the top H in hopes of hitting partner's strong suit. A S is my last choice because there is too much risk that dummy might have ATx or KTx of S, and my S lead could give away the setting trick. With this hand, I lead the C5 in hopes of taking 3 Cs plus a S plus a D.

Any suit can always work out best. Just that a minor suit lead has a much smaller probability to be the winner. At least it looks so to me :)

At IMPs a lead stands out a mile. At MPs I consider a more passive lead, then probably lead a anyway.
