You win the small spade lead with the queen. When you play a heart from either hand, the king wins but West shows out.

I doubt that you can pick up the suit with only one loser, so I would plan for two and the A to the defense. At trick 3, I lead the Q in hopes of trapping the stiff J in East. If the J is stiff in East, and if East also has the A, then I can claim 10 tricks.

Yes, but no club holding helps me if this is the case, I always have a club loser, not sure if 5 spades guaranteed, but 4054 is probably my best chance.
