1D is ostensibly natural, but partner tends to fake this bid sometimes with hands that don't have good 4 card majors. What say you here?

3H if that would be understood as a splinter. 3D, otherwise.
If 1D tends to deny a good 4 card major, why not 1N by responder? It's not like we're going to end lower than that anyway. Makes your rebid here a bit easier, too. (because you'll open 1D instead of 1C)

There's no harm in rebidding 2♣ - the opponents have ten hearts or nine very good hearts at minimum, they would have to be incredibly dopey to not balance. Then you can bid 3♦. Now assuming he only fakes this bid when he has a decent hand (maybe he doesn't want to bid 2NT or the like without both majors stopped) - he should have a good idea of the sort of hand you have (you would probably double with 3 card diamond support - and a bad hand with 4 card diamond support would have bid 2♦ the first time) and place the contract appropriately.

If you bid 2C/D, LHO bids 2H and partner bids 3H.
If you bid 3D, LHO bids 3H and partner doubles.
If you bid 3H, LHO doubles, partner passes.
If you bid 3D, LHO bids 3H and partner doubles.
If you bid 3H, LHO doubles, partner passes.

How many clubs did you show first time ?
I'd bid 3♥ which would show a 4045 or 3046 in our methods, hands with a singleton are bid differently, and I think I'd make the same bid playing more standard methods.
Pass over the X of 3♥ would show a really bad hand in context opposite a known heart void for us, so probably less than 7 working points and not a real load of diamonds. I'd bid 3N to show a minimum ish 3046 with a spade stop not expecting it to end there unless partner has a lot of heart values, but allowing partner to bid 4m to choose the suit.
